YM Forms and Registration

Youth Ministry Registration 2026-2027

• REGISTRATION OPENS JULY 1, 2026 AND ENDS AUGUST 31, 2026. All programs begin Sunday, September 20, 2026.

• You must register every year you would like to participate, including for Year 2 of sacrament preparation.
• All registrations must be completed online, if you need assistance please contact a Youth Ministry staff member

• $100 per child/youth
• Incomplete forms or those missing proof of payment will not be accepted
• Payments may be made online, in-person, or via mail for checks - see information below  
• Registration fees are waived for children of adult full-time volunteers.
• No family will be excluded due to inability to pay. Please contact a Youth Ministry staff to inquire about financial assistance.
• Payment plans and installments MUST be arranged in person

• Unpaid balances from past years must be paid before we accept new registrations.  

• 50% of total amount paid will be refunded if notice of withdrawal is received by STMYM staff before SEPTEMBER 20, 2026.
• NO REFUNDS will be given after September 20, 2026 regardless of attendance in the program.
